Connected Health COTM Revisit

HIRANI featured Northern Ireland’s leading independent provider of premium private homecare services, Connected Health, in our Company of the month feature in early 2023. Over 2 years later, Connected Health has achieved several milestones and has expanded their workforce to over 1900 carers, supported by regional and central teams including tech, analytics, and care operations.

Most notably, Connected Health were honoured with the Team of the Year – Large Business award at the 2025 Irish News Workplace & Employment Awards. The award recognised the cross-functional achievements of their IT and operational teams in building a tech-enabled model of domiciliary care. Connected Health have also completed the full rollout of their in-house care delivery platform, Connexions, across the island of Ireland marking a new era in intelligent care coordination.

Connected Health have invested in restructuring to create more agile, cross-functional teams that can better support digital transformation, training, and innovation at scale. A strong emphasis on internal development pathways and workforce digitisation has also helped them optimise care delivery and support services across Ireland.

Introducing several new innovations, the most significant being Connexions, their digital care management platform designed entirely in-house. Supporting rota generation, real-time care insights, reducing admin time, and allows for proactive, outcomes-focused service delivery. Connected Health have also launched pilot initiatives through their Living Lab including wearable technologies, medication-dispensing robots, and virtual monitoring; bringing next-generation tools into real-world care environments.

Connected Health’s infrastructure and tech-enabled care model have allowed them to expand geographically and into new service areas. Broadening their reach across underserved rural regions and actively pursuing partnerships that support complex care pathways, including hospital-at-home, reablement, and social prescribing.

The ongoing pressures on health and social care; staffing shortages, rising demand, and post-pandemic complexity have tested the resilience of Connected Health. But they overcame these pressures by investing in technology, training, and flexibility. The Get Online Scheme was launched during COVID-19 to reduce isolation among digitally excluded clients, while Connexions enabled Connected Health to streamline care delivery and improve staff efficiency and satisfaction.

The sector-wide shift toward digital transformation and integrated care has accelerated their strategy. By working closely with third-party systems, Connected Health have built infrastructure to adapt quickly to regulatory, demographic, and economic pressures. Their focus on preventive, tech-powered care has helped us maintain both quality and agility.

Connected Health shared with us their goals for the ahead couple of years, “Our goals include expanding our hybrid homecare model, enhancing data analytics capabilities to personalise care plans, and continuing to reduce environmental impact through more efficient workforce routing. We’re also deepening our investment in staff development, aiming to create more digital champions within frontline care teams.”

The team at Connected Health shared they have several exciting launches coming up. They are currently scaling Connexions features, integrating AI to support early risk detection and care personalisation and also preparing to launch new collaborative projects through the Living Lab, with upcoming trials on smart home tech. In addition, they will soon announce new geographic and service line expansions that will further position Connected Health at the forefront of health innovation across the island of Ireland.
